Immunization Requirements

In order to register for six or more credits, proof of immunity against measles, mumps, rubella (MMR), and meningitis is required. Students may decline meningitis vaccination by completing and submitting a signed response form.

Requirements of attendance at a post-secondary institution regarding immunization for measles, mumps, rubella and meningococcal disease are mandated by New York State Public Health Laws 2165 and 2167, and Title 10 New York Codes, Rules and Regulations Subpart 66-2 (10 NYCRR Subpart 66-2).

This Immunization Record Form outlines the specific immunization requirements that must be met.

Please call Student Health Services at (212) 650-8222 to confirm that your records were received. Even if you mail or fax your records, please be sure to bring a copy of this important information with you on the day of registration. 

PLEASE NOTE

Failure to submit required documents will result in a WA grade (Administrative Withdrawal) for all your classes. You will lose credit for all currently registered courses, and will remain liable for all tuition and fees. If you received financial aid, it can be suspended.

To avoid these costly penalties, please submit proof of your immunization records or obtain the MMR vaccine (at no cost to you) at the Student Health Services office.
