POLICIES AND PROCEDURES

The initial ID card is free. Replacements for lost or stolen cards is $5.00. Pay at the Bursar first then bring the receipt with "6 Points of Identification" to the ID Office, NAC 1/206.
The Public Safety Department has implemented a standard where people must present "6 Points of Identification" before we issue them a CCNY ID card. We came up with this procedure to standardize the existing unwritten practices we had in place in the ID office and to minimize the chance of issuing fraudulent ID cards which could lead to incidents of identity theft.
The concept of the "6 Points of Identification" are in compliance with the intent of the Homeland Security Act. Many of the documents accepted as proof are the same as required by the New York State Department of Motor Vehicles and the US State Department Passport Division.
The procedure is that people will have to provide proof of identity. Their identity documents are assigned a point value from 0 to 6. A combination of 6 or more points are required before we issue an ID card. The pieces of identification that we will accept are diverse enough that all applicants should be able to come up with at least 6 points.
YOU WILL NEED 6 POINTS OF IDENTIFICATION TO OBTAIN A CCNY ID CARD
EXPIRED DOCUMENTS ARE NOT ACCEPTED UNLESS INDICATED
ALL DOCUMENTS MUST BE ORIGINAL OR COPIES CERTIFIED BY THE ISSUING AGENCY
YOUR DOCUMENTS MUST ALL BE IN THE SAME NAME AND THE NAME MUST MATCH THE NAME
