Reports to Dean of Student Affairs,

(Matters involving student conduct)

Reports to Staff, Faculty, Student Advisors, Assistant Dean for Diversity/other Deans, Executive Director of Counseling

Reports to CCNY Office of Diversity, Affirmative Action and Compliance

Reports to Campus Public Safety/Police

Confidentiality assured

 

-    Anonymous  or non-anonymous report may be submitted via

     https://www.ccny. cuny.edu/csom/mistreatment-policy), Professionalism Documentation Form, or emailed to mistreatment@med.cuny.edu

 

 May be reported and adjudicated through the Honor Code process

 

Associate dean for student affairs, in discussion with the dean and/or  assistant dean for diversity determine if referred to Office of Diversity, Affirmative Action and Compliance, Police/ Public Safety, and/or other committee (e.g. Student Honor Code Committee)

Confidentiality assured

 may be forwarded  to Faculty-Student Disciplinary Committee, CCNY Office of Student Affairs for resolution or action

 

Information regarding any investigation and/or outcome may or may not be shared with Dean

Confidentiality assured

 Report is reviewed  by Police and/or  Office of Public Safety  for investigation and/or determination of next steps 

 

Information and/or outcome of reported incident may or may not be shared with Dean
