Forms (2020-2021)

The current forms, if required, are available via student's "To-Do-List" on CUNYfirst as well as in the Office of Financial Aid.

  • Please read the instructions on the forms carefully before completing the form Students can submit their signed documents through the Secure Portal or bring in your signed supporting documents.
  • Follow-up with your CUNYfirst To-do-list and check your student email.
  • Any forms, request or consent completed with lead pencil or incomplete forms will not be accepted.​
  • Electronic signature, or e-signature will not be accepted.
  • Parent's signatures are also required for dependent students.

Federal Direct Loan Forms
Undergraduate Students

Undergraduate students must submit a Direct Loan Application via CUNYfirst. In order to navigate and complete the application you must have a FAFSA on file for the corresponding year.

  1. Log onto your CUNYfirst account and click on "Student Center".
  2. On the Student Center page, navigate to the Direct Loan Processing form and link.
  3. Fill out a Direct Loan Processing form.

(To cancel, reduce or request for an additional loan after completing the application CUNYfirst. Please fill out the form and submit it to the financial aid office.)

Graduate Students

All matriculated students in a Masters or Ph.D. program, with a valid 2019-2020 FAFSA, will automatically be “packaged” with a Federal Direct Unsubsidized student loan each academic year.

Graduate students will be required to “Accept” their loan via the “Accept/Decline”.

  1. Log onto your CUNYfirst account and click on "Student Center".
  2. On the Student Center page, navigate to Finances
  3. Click on Accept/Decline Awards.

(Prior to accepting the loan, a graduate student will have the option to decrease the amount of unsubsidized loan)

The Financial Aid Office is pleased to announce our new secure portal effective Friday, March 13, 2020.


Advantages of this new portal:

  1. Faster and secure way to submit required document(s).

  2. No need to be on campus or wait in line at the Financial Aid Office to submit your required documents.

  3. Documents are auto date stamped upon submission.

*To protect your PII (Personally Identifiable Information) Any email attachments sent to the Financial Aid Office outside of this new portal will be immediately deleted without review.*
