Each year when you apply for financial aid, you may be selected for a process called verification. This process is required by the U.S. Department of Education to ensure the accuracy of financial aid applications. In the past, students selected for verification had to collect specific forms and tax documents and submit them to the campus Financial Aid Office in person, via fax, or by mail. Now, students can easily complete their forms and upload documents online from their phone, tablet, or computer!
What is Student Forms?
Student Forms is our new student portal that allows students (and parents) to electronically sign and upload documents! This means you can securely submit your financial aid documents without visiting the campus Financial Aid Office. As students and financial aid staff work through the forms, the Student Forms software provides real-time updates on any additional documentation needed based on the answers provided. Once you have submitted all required forms to resolve outstanding financial aid tasks, the integration with other CUNY applications will update CUNYfirst in real time.

Please allow 6–8 weeks for processing verification documents submitted via Student Forms.
Students living at The Towers can upload their Towers form to our secure online file drop or submit it in person.

How To Request a Loan Application
Step 1: Create or log into your existing account at https://cuny-cty.studentforms.com/
Step 2: Once you have successfully created your account, you will be able to self-initiate your loan request electronically.
Step 3: On the top right corner click the “Manage Request” button to view all the available loan application options.
Step 4: Please refer to the Request Type column and click on the corresponding + button for the application that you would like to request.
Step 5: After you click the + button, please follow the instructions on the following window accordingly.
Step 6: Once your request has been successfully created, you must click the “ok” button.
Step 7: At the next window, click on the (>) arrow to begin the process

- Federal Loan Direct Status Change Form is the only form that requires an E-Signature.
- All other loan applications must be downloaded, printed, and have a “wet” signature (pen) before uploading.
Resolving Error Codes 1010
Error Code 1010 indicates that the student is entering information into the confirm information section (First Name, Last Name, DOB, and SSN) that does not match what is on the ISIR. (Please review your FAFSA Application)
Common data entry errors include:
- Not including suffixes in the last name that were included on the FAFSA. i.e. Evans Jr
- Using a nickname or shorter version of full name, when full name was used on the FAFSA Rich instead of Richard
- Extra spaces were entered between names on the FAFSA but not during account creation McFadden was entered Mc Fadden on the FAFSA.
- Special characters entered in name on FAFSA, and not entered when creating an account. i.e. La'Juan.
Note: Users of Apple devices may experience issues creating an account if they have smart punctuation enabled on their device. Smart punctuation will need to be turned off prior to creating an account.
Please contact our Financial Aid Office if the problem is not resolved after reviewing your data on the processed FAFSA Application called an ISIR /Student Aid Report.
Student Forms Not Opening in Your Browser
You must allow the Student Forms website to open pop-up windows. Below are instructions for popular browsers to help you resolve this issue.
Chrome – How to Change Pop-up & Redirect Settings

On your computer, open Chrome.

Go to a page where pop-ups are blocked.

In the address bar, click Pop-up blocked.

Click the link for the pop-up you want to see.

To always allow pop-ups for the site, select Always allow pop-ups and redirects from [site], then click Done.
Safari – How to Allow Pop-ups

In the Safari app on your Mac, choose Safari > Preferences, then click Websites.

Click Pop-up Windows.

To block or allow pop-ups for:

A specific website in the list: Select the website in the right column, then choose the option you want.

All other websites: Click the When visiting other websites pop-up menu at the bottom right, then choose the option you want.

You can see all customized websites under Configured Websites. If you don’t see this list, you either haven’t customized any sites or you’ve cleared the list.

To allow pop-ups for all websites, make sure no sites are listed under Configured Websites (select and click Remove to clear the list), then choose the desired option in the When visiting other websites menu.
Firefox – How to Allow Pop-ups
When Firefox blocks a pop-up, it displays an information bar (unless previously dismissed) and an icon in the address bar.

Click the Options button in the info bar or the icon in the address bar to open a menu with these choices:

Special Circumstances Appeal Information
During the academic year, you or your family may experience a significant change in financial circumstances that reduces your income compared to what was reported on the FAFSA.
If your income has decreased, you may qualify for additional federal financial aid. FAFSA uses income information from two years prior, so if you previously didn’t qualify for a Pell Grant, you might now be eligible based on an income adjustment. Common reasons include unemployment, death of a wage-earner, divorce, or loss of untaxed income (e.g., child support). To review your case, you’ll need to provide income details and supporting documentation.
Each case is unique and reviewed individually. Submitting an appeal does not guarantee approval. While awaiting a decision, consider alternative payment options.
We encourage you to contact the CCNY Financial Aid Office to discuss your situation and determine if you qualify to file an appeal. Schedule an appointment with a Financial Aid Administrator.

Satisfactory Academic Progress
Students that are NOT meeting SAP will have SAP forms posted on to your student forms account. If you are NOT making SAP and you do not see the SAP forms student forms account. Please contact the financial aid office to have our staff manual post your SAP forms. Please review CCNY website for deadlines to complete the SAP process.
Satisfactory Academic Progress Standards for Title IV Financial Aid
All students must satisfy the following qualitative and quantitative academic standards in order to remain eligible for federal financial aid. These qualitative and quantitative standards refer to the following Title IV Federal Aid programs: Federal Pell Grant, Federal Supplemental Educational Opportunity Grant (FSEOG), Federal Work-Study, and William D. Ford Federal Direct Loans.
Students will be measured against the Title IV Satisfactory Academic Progress Standards at the end of each spring semester in order to determine eligibility for the upcoming year. Those who fail to meet the academic standards will have their federal aid automatically suspended until they meet the minimum standards.
PLEASE NOTE: Students who can document that extraordinary or exceptional circumstances resulted in their failure to satisfy academic requirements may be able to apply for a waiver to receive federal aid for another semester.

Dependency Appeal
Under certain conditions, the Financial Aid Office is authorized to override a student's dependency status. To qualify for this appeal, the reason for separation from your parents must be due to an unusual, unsafe, and/or detrimental family situation.
Before you submit your request, please note the following:
None of the conditions listed below qualify as unusual circumstances meriting a dependency override:

Parents refuse to contribute to the student’s education.

Parents are unwilling to provide information on the FAFSA or for verification.
-
Parents do not claim the student as a dependent for income tax purposes.

Student demonstrates total self-sufficiency.

If you believe your educational expenses are higher than CUNY’s "Cost of Attendance" (budget), you can request a special form.
