Address Change
Employees are required to notify the Human Resources of changes to their home or mailing address. An outdated address may prevent important documents related to payroll, taxes, benefits, and other employment matters from being delivered accurately and on time. When notified, the Office of Human Resources will update the payroll, health insurance, and the transit benefit system, if applicable.
CUNYFirst Employee Self-Service: Update Your Address
Employees can update their address using the CUNYFirst Employee Self-Service system by following the steps listed below:
- Log in to CUNYFirst
- Select “Employee Self-Service”
- Click on “Personal Details”
- Select “Addresses”
- Select the address to modify
- Edit the updated address details
- Click “Save”
Note: Address changes made through CUNYfirst Employee Self-Service are reflected only in CUNY systems. Employees are responsible for notifying the appropriate union, pension fund, and Benefits Unit of any address changes, as applicable.
Legal Name Change
CUNY requires legal documentation for any change in legal name.
Employees must notify Social Security Administration (SSA) of any legal name change and provide a copy of the updated social security card to HR for payroll purposes.
Failure to update your legal name and Social Security information may result in discrepancies on your paycheck or paystub, Form W-2 Wage and Tax Statement, and other university records or systems that require a current Social Security card.
Faculty and staff may request to update their university records to reflect a new or resumed legal name in accordance with a court-ordered legal name change.

Change of Beneficiary
Employees are encouraged to review their beneficiary designations regularly, particularly after major life events such as marriage, divorce, the birth or adoption of a child, or the loss of a loved one.
Employees should also notify their retirement plan provider and, if applicable, their union welfare fund benefits office of any changes to their contact information or beneficiary designations.
Retirement and Welfare Fund Information
- DC-37 members: Complete your Beneficiary Designations Form to ensure that the beneficiaries on file are accurate and return it to DC-37.
- PSC CUNY members: Complete the Death Benefit Beneficiary Designation Card and submit it to benefits@ccny.cuny.edu via secure transfer
- TRS NYC members: To make changes to your beneficiaries or designated guardians, log in to the TRS NYC Member Portal.
- NYCERS: To make changes to your beneficiaries or designated guardians, log in to the MyNYCERS account and select “Manage Beneficiaries” in the menu.
- TIAA: To make changes to your beneficiaries, access your account online at tiaa.org or complete and submit their TIAA Beneficiary Designation Form (F11468) and return it to TIAA.
- NYS Deferred Compensation Plan: Complete the NYSDCP Beneficiary Change Form and return it to NYSDCP.
- NYS 529 College Savings Program members can update their beneficiaries online: Direct Plan Account

Change of Emergency Contact
Employees can update their emergency contact information through CUNYfirst Employee Self-Service. Please review your emergency contact information regularly and keep it current.
CUNYFirst Employee Self-Service: Update Your Emergency Contacts
Employees can update their Emergency Contacts using the Employee Self-Service system in CUNYFirst by following the steps listed below:
- Log in to CUNYFirst
- Select “Employee Self-Service”
- Click on “Personal Details”
- Select “Emergency Contacts”
- To update an existing contact, select the contact you want to modify. To add a new contact, select the + (Add) button.
- Edit the contact details
- Click “Save”
Preferred Name Change
Preferred names are limited to first and middle names. A preferred last name may not be selected.
A preferred name may be requested for use in college e-mail, and unofficial documents such as college directories, course catalogs, and other internal documents.
Please note that a preferred name will not be used for payroll, benefits, or employment verification purposes.

Employment Verification
Employment Verification Request Form
Employment Verifications are processed in accordance with CUNY’s policies and procedures, and all applicable federal, state, and local laws. An employment verification typically confirms an employee’s basic employment information, which may include: employer name, address, location, job title, employment status, and most recent hire date.
Release of Employment Information
To protect the confidentiality of employee personnel records, employment information generally will not be released without the employee’s authorization, except as permitted or required by law.
Information may be released to CUNY and certain authorized governmental agencies, including the Office of Human Resource Services Administration and the Department of Health, Education, and Welfare, or in response to a valid subpoena or other legal requirement.
